NURSING:
Registered Nurses (RNs) and Licensed Vocational Nurses (LVNs) work closely with your doctor. They provide the ongoing assessment, skilled treatment, and support you through your recovery requirements at home.
PHYSICAL THERAPY:
Registered physical therapist (PTs) and registered physical therapy assistants (PTAs) assist patients in strength training, balance, mobility and gait training, home safety, instruction on assistive devices.
OCCUPATIONAL THERAPY:
Registered occupational therapist (OTs) and certified occupational therapist assistant (COTAs) provide assessment and treatment to develop, recover, or maintain patients’ ability to perform activities of daily living.
MEDICAL SOCIAL WORK:
Help patients understand challenges related to care at home.
Help coordinate community resources and services available to patients at home.
SPEECH THERAPY:
Speech-language pathologists (SLP) provide evaluation, diagnosis and treatment of communication disorders, cognition, voice disorder, and swallowing disorders.
HOME HEALTH AIDE:
Certified nurse’s aides (CNAs) assist patients in home as needed with important activities such as personal care, hygiene, grooming, and other activities of daily living (ADLs).
